The Salt Lake Chamber Small Business Awards recognizes small businesses for their continued growth, innovation, and community involvement.  These awards will be presented at the Salt Lake Chamber Annual Meeting on February 11, 2010.

• If you are not a representative of the applicant or do not feel you have enough information to complete this form, please fill out a nomination form. We will contact the applicant to collect additional information. (Nominations due by December 18, 2009)
   
• The selection committee will determine the award given (Small Business of the Year, Entrepreneurial Success Award, Minority Small Business Award or Community Service Award)
