Salt Lake Chamber of Commerce :: Small Business Awards

The Small Business Awards recognizes small businesses of the Salt Lake Chamber for their continued growth, innovation, and community involvement.

We appreciate you taking the time to nominate a small business for one of these awards. Your comments will provide us with an invaluable look at into the nominee. The deadline to submit this nomination form is December 18, 2009.

The selection committee will determine the award given (Small Business of the Year, Entrepreneurial Success Award, Minority Small Business Award or Community Service Award)
